Diabetes Prevention and Management Collaborative (APCC)
A new “wave” of programs focusing on Diabetes Prevention and Management is planned and there may be an opportunity for practices from this Division to participate. Participants will receive hands-on support, the ability to monitor improvements via the Improvement Foundation web portal, and the opportunity for sharing ideas and experiences with colleagues, as well as $4,000 towards costs and any travel for workshops will be subsidised.
