Referrals Wanted - Life! Taking Action on Diabetes

Recent changes to the eligibility criteria for Life! courses now enables community members to self refer without requiring blood results to enter the course.  This will benefit our general practices by creating less administration and also increases the number of participants in our courses.  Referrals are however still welcome on an ongoing basis from our practices for eligible patients.

• Croydon: Course commencing 7^th June  2010 @ Eastern Ranges GP Association, 23 Maroondah Highway, Croydon
• Croydon: Course commencing 8^th June 2010 @ Eastern Ranges GP Association, 23 Maroondah Highway, Croydon
• Lilydale: Course commencing 13^th June 2010 @ Eastern Ranges Allied Health Site, 2F John Street, Lilydale

Please note that additional courses are being scheduled in Healesville and Pakenham.
