After Hours GP Clinic: Eastern Ranges After Hours Medical Service (ERAHMS)

The Eastern Ranges GP Association (ERGPA) operates After Hours Medical Services in Healesville, Yarra Junction and Upper Ferntree Gully (co-located with Angliss Hospital). The Eastern Ranges After Hours Medical Services are open to the general public needing to see a doctor for urgent medical matters that cannot wait until the next day.

ATAPS -  Better Outcomes in Mental Health Care (BOiMHC)

ATAPS enables GPs under the Better Outcomes in Mental Health Care (BOiMHC) program to refer consumers to allied health professionals who deliver focussed psychological strategies.

Australian Primary Care Collaboratives (APCC)

The APCC Program helps general practitioners (GPs) and primary health care providers work together to improve patient clinical outcomes, reduce lifestyle risk factors, help maintain good health for those with chronic and complex conditions and promote a culture of quality improvement in primary health care.

Clinical Audit Tool (PEN CAT)

Cat allows you to provide high quality patient care, be in control of your own patient information, identify patients in high risk target groups, improve practice revenue and meet accreditation requirements.

Close the Gap

The Close the Gap program aims to reduce and then close the gap in Aboriginal and Torres Strait Islander life expectancy by encouraging greater use, and improved cultural safety, of mainstream primary care services for Aboriginal and Torres Strait Islander individuals and communities.

Continuing Professional Development (CPD) program

ERGPA’s CPD program offers a wide range of quality educational events to GPs and Practice Staff and is responsive to the expressed needs of local GPs.

Diabetes - Education

ERGPA offers services for people who are at risk of developing Type 2 Diabetes Mellitus or are currently diagnosed with Diabetes of any type.

Flu Vaccination

Eastern Ranges GP Association (ERGPA) is a Victorian provider, which assists all business types, regardless of size or industry, by providing excellence in customer service and provision of vaccines. These are administrated by highly skilled and Accredited Nurse Immunizers.  ERGPA is fully compliant with Quality Improvement Council and has a valid permit (certified by Department of Health, Victoria) to provide such a service.

General Practice Liaison (GPL) Program

Our General Practice Liaison Program (GPL) has been established in conjunction with Eastern Health to improve communication between General Practitioners and hospitals.

Immunisation

For ERGPA the major focus of the immunisation program is to support General Practices and encourage participation of General Practitioners in the General Practice Immunisation Incentive (GPII) scheme as part of the National Immunisation Program (NIP).

Life! Diabetes Prevention

The Life! program is directly based on the Finnish diabetes prevention program which has been proven to reduce the incidence of newly diagnosed diabetes by up to 58% in course participants.

The Life! Program focuses on Victorians aged 50 years and over, and Aboriginal and Torres Strait Islander people of all ages at risk of diabetes. The main aim of the program is to prevent people from developing type 2 diabetes and to contribute to early diagnosis in those who have type 2 diabetes but they are unaware.

Mental Health Nurse Incentive Program

The Mental Health Nurse Incentive Program provides a non-MBS incentive payment to community based general practices, private psychiatrist services and other appropriate organisations (such as Divisions of General Practice) who engage mental health nurses to assist in the provision of coordinated clinical care for people with severe mental disorders.

NPS (National Prescribing Service)

The NPS program offers one-to-one and group training with ERGPA’s Pharmacist Facilitator who provides up to date information on drug therapies. The program also provides access to small group discussions, workshops and clinical audits, providing increased access to the PIP QPI Initiatives.

Post Traumatic Stress & Bushfire Counselling Service

The Post Traumatic Stress & Bushfire Counselling (PTS&BC) Service provides individual, family and group counselling to people living in the Bushfire affected areas.

Practice Health Atlas (PHA)

The Practice Health Atlas (PHA) is a decision support tool. PHA is a generated report which enables General Practice teams to visually analyse their patient population and reflect on their activities and to develop innovative business models for more effective health care services/ outcomes.

Prescribing Data in General Practice Demonstration Project (PDGPD)

The Prescribing Data in General Practice Demonstration (PDGPD) project is a quality improvement (QI) activity for GPs. The project has been developed by the National Prescribing Service (NPS) in conjunction with the Australian General Practice Network (AGPN). Up to 180 practices and 20 Divisions across Australia will be selected for the project.

Eastern Ranges GP Association is pleased to offer a range of Business Management courses. Aimed at primary health care providers and their staff, to help in setting up their own business and make informed decision to give their business the best chance of success.

Eastern Ranges GP Association (ERGPA) offers support and advice to practices seeking to recruit GPs and practice nurses.

Rural Primary Health Service (RPHS)

The Rural Primary Health Service program provides allied health services to patients living in the rural areas of the Division catchment including the Yarra Valley and the Warburton Valley.

Smart Practice 2 is a new electronic version of Smart Practice 1 (e-version).  This exciting program contains a number of useful and innovative resources which can be downloaded to enhance the day to day running of your practice.

Student Nurse Placements

Eastern Ranges GP Association (ERGPA) has partnered with Monash University to facilitate student nurse clinical placements throughout the division. Student clinical placements are a valuable recruitment tool and contribute to training the next generation of practice nurses. They also provide links to education providers and innovation, help foster a culture of learning, and they assist with practice nurse duties. ERGPA provides assistance with supervisor (preceptor) training, support and administrative assistance.
