Australian Better Health Initiative (ABHI)
The ABHI project aims to promote solutions to primary care integration between general practice and other local health providers that will asssist in the delivery of more ‘seamless’ patient care.
The project aims to improve chronic disease prevention and management by improving service coordination , partnerships and linkages between primary health providers; better utilisation of tools for management of patients which a chronic disease, and developing local chronic disease care pathways and mapping local service provision for purposes of service improvement through coordination and referral processes.

Many networking and educational events have been run by the program, giving local GPs and staff and allied health providers an opportunity to create and strengthen linkages. The directory ‘Get Connected. Get to Know’ further promotes local relationships.
