Diabetes Prevention (Lifestyle Modification Program)
Lifestyle Modification Programs (LMPs) are designed for those at high risk of developing Type 2 diabetes. With participation in a Lifestyle Modification Program research has shown Type 2 diabetes can be prevented in up to 58% of cases.
ERGPA LMP courses are delivered in Croydon, Lilydale, Healesville and Pakenham.
Diabetes Prevention 50+

Referring 50+ patients to Life! Courses - Eligibility Criteria
- Score 12 or above on the Australian Diabetes Risk Tool
- Are 50+
- Have not already been diagnosed with Type 2 diabetes
The course involves six, 90 min sessions which aim to improve nutrition and physical activity over an eight month period. Participants will be supported by trained health professionals, including a Dietician, Diabetes Educator and Exercise Physiologist to make positive lifestyle changes. The program is funded by Diabetes Australia-Victoria and is available free of charge for eligible patients in Croydon, Lilydale, Healesville and Pakenham.

Diabetes Prevention 40-49 years
Patients 40-49 who score greater than 12 on the Australian Diabetes Risk Tool are eligible to be referred to a fully funded Lifestyle Modification Program.
The course involves six, 90 min sessions designed to improve nutrition and physical activity over an eight month period. Participants will be supported by trained health professionals, including a Dietician and Exercise Physiologist to make positive lifestyle changes.
Changes to the MBS Schedule in May 2010 enable GPs to claim Item No.707 for Prolonged Health Assessments when assessing patient suitability.

Identifying patients at risk using the PEN Clinical Audit Tool
Diabetes Australia - Vic and the General Practice Victoria Program team has recently completed development of a new resource using the PCS Clinical Audit Tool (CAT), a data extraction tool that can make systematic diabetes prevention and management easier. The CAT allows you to quickly and effortlessly look at a snapshot of practice data to get valuable information such as:
- Patients at high risk of type 2 diabetes and eligible for referral to ERGPA's Life! Taking Action on Diabetes and Reset your Life Courses (40+)
- Number (and names) of patients with diabetes
- Patients who are at target levels for HbA1c, blood pressure and cholesterol
- Diabetes SIP items remaining and progress towards completion of the annual cycle of care
This evidence-based resource will guide you through step by step instructions for using the CAT for effective prevention and management of diabetes. There are screen shots for easy comparison with practice software, activities to try out new skills and plenty of spaces to take notes while working through the various sections.
