Home Medicines Review (HMR)
The HMR program provides a Pharmacist Facilitator to work with GPs and Pharmacies to initiate the Home Medicines Review process.
Services supported through the HMR program include support in implementing home medicines review through your practice and support in using information technology to simplify the HMR process.
The HMR program provides GPs and practice staff with advice and support from an accredited Pharmacist in this worthwhile PBS Item number 900, as well as the opportunity to create vital networking links with local Pharmacists.
