Life! Taking Action on Diabetes
The Life! Program focuses on Victorians aged 40 years and over, and Aboriginal and Torres Strait Islander people of all ages at risk of diabetes. The main aim of the program is to prevent people from developing type 2 diabetes and to contribute to early diagnosis in those who have type 2 diabetes but are unaware.
To find out if your patient is eligible for the Life! course encourage them to complete the Australian Type 2 Diabetes Risk Test (AUSDRISK), available under Publications and Resources, or by calling Sally Ingram on 9871 1000
Refer your patient to the Life! course if they:
- Score 15 or above on the diabetes risk test
- Are aged 40 years or above, or are an adult Aboriginal or Torres Strait Islander person
- Do not have type 2 diabetes
Two referral forms are avaiable on our website, one for patients aged 40-49yrs of age, the other for patients over 50, or by calling Sally Ingram on 9871 1000
The course involves six, 90min sessions which aim to improve nutrition and physical activity over an eight month period. Participants will be supported by trained health professionals, including a Dietician, Diabetes Educator and Exercise Pysiologist. The program is available state-wide and it is free for people on low-middle incomes.
